To be eligible for this credit, the impairment must be significant enough to markedly restrict the individual's ability to perform a basic activity of daily living, or the individual must require extensive therapy to sustain a vital function. The impairment must also be prolonged, meaning it has lasted, or is expected to last, for a continuous period of at least 12 months.
The form itself is divided into two main sections:
1. Part A - which must be completed by the person with the disability or a legal representative. This part provides personal information about the individual applying for the credit.
2. Part B - which must be filled out by a qualified practitioner, such as a medical doctor, optometrist, audiologist, occupational therapist, psychologist, or speech-language pathologist, depending on the nature of the impairment. This section requires detailed medical information about the nature of the impairment, how it affects daily living, and the duration of the impairment.
The completed T2201 form must be submitted to the Canada Revenue Agency (CRA) for approval. Once approved, the individual may claim the disability amount on their income tax and benefit return. The approval can also open the door to other government programs and benefits for those with disabilities.
