Referring a patient to Monarch Health

Monarch Health provides proactive assessment, care coordination and support at home as uninsured services alongside a patient's existing publicly funded care. We do not take over primary care, and we do not create a parallel route to insured services.

What we can take on

  • Proactive assessment beyond what insured care typically provides.

Comprehensive assessment, functional measurement and structured prevention planning for patients who want more depth than insured care provides.

  • Care coordination for complex patients

Where several providers are involved and coordination is consuming your time and the family's, we can hold that thread.

  • Support at home

Nursing care, personal care and home safety work for patients whose situation makes staying at home what matters most.

  • Functional review over time

Repeat measurement of strength, balance, gait and day-to-day function, reported back to you. Monarch communicates relevant findings, priorities, interventions and changes over time back to the referring physician, with the member's consent.

The boundaries, stated plainly

  • We do not replace the referring physician or assume primary care responsibility unless that is separately arranged.
  • We cannot expedite MSP-covered investigations, diagnostics or specialist referrals, and we will not present ourselves as able to.
  • Membership is never a precondition for a patient receiving insured physician services.
  • Where a patient is eligible for publicly funded home and community care, we help them access it rather than substituting for it.
